Form 114a - Declaration of Fitness to Parachute for a Solo Parachutist

Form 115 - Tandem Declaration of Fitness (Only use if over 54 years of age or if Form 114a is unsuitable)


Parental/Guardian Consent Form (To be used by any person under the age of 18)

Parachute Centre Membership Agreement (Used by club members)

 

Please note:

The maximum age for attending the SOLO Parachute Training Course is 54.

The maximum age for the Tandem Skydive is 70.

 

Form 114a (Solo declaration of fitness) can be used for both Tandem and Solo.

Form 115(Tandem declaration of fitness) (Less stringent) can be used for the Tandem Skydive only.

 

After doing a Tandem Skydive you may fancy attending the Solo Course and if you have paid your doctor for a Tandem Medical you will have to go back and to get the correct form signed.
